Dr. Gary Lindemoen and Associates accepts patients on referral from dentists and physicians. When your dentist or physician refers you to OMS Associates, you may make an appointment yourself or your dentists or physicians office may make an appointment for you. Your appointment will be made with the surgeon to whom you were referred, to the surgeon of your choice or at the first available appointment that meets your schedule. Please have your dentist or physician send us your most current X-rays to start the process of establishing a patient chart for you.
The nature of our practice is to give our patients the utmost in care and service. We will make every effort to maintain our office schedule and see you at your appointment time. However, the time to complete a surgery is often unpredictable. Please excuse any delays and know that we will give you the same careful attention as soon as it is possible.
If, for some reason, it becomes necessary for you to change your appointment, please contact our office at 701-232-9565 or 800-235-9565 to cancel or reschedule. We require a 48-hour advance notice for scheduling changes. Patients who cancel and reschedule an appointment multiple times may forfeit the possibility of another appointment. A $25.00 fee may be assessed to those who fail to give a proper cancellation notice.
A pre-surgical estimate is an estimate of your costs and insurance coverage that is prepared for you prior to our appointment from your X-ray and planned procedure. While we try to anticipate all costs related to your surgery, your course of treatment may vary based on the doctors diagnosis and necessary procedures performed on the day you are seen. While we will work to facilitate prompt payment from your insurance company, you remain responsible for the charges incurred during your visit. We will collect the co-pay and coinsurance amounts estimated on the day of service. Accounts without insurance will need to be paid in full on the day you are seen.
While we welcome children as patients, you should not bring non-patient children to the office. We recommend and encourage you to make arrangements to have your child or children cared for at home or in a daycare facility as a consideration to other patients and because your children will be happier and more comfortable somewhere other than our waiting room.
All surgical patients planning to undergo sedation or general anesthesia must have an escort accompany them to their appointment. The escort must be present before surgery and remain in the office until the patient is discharged. The escort is the individual who will provide transportation for the patient after surgery and be given post-op care instructions. Patients receiving local anesthetic do not require an escort.
