Welcome to our office! We would like to take this opportunity to help you understand our office billing and insurance policies and to help you obtain the maximum benefits allowable under your policy.
The financial obligations for the treatment we render to you are your responsibility. We welcome and encourage questions regarding services and fees prior to treatment to avoid any misunderstandings.
We ask for payment in full at the time of service. Prior to your appointment, an estimate of anticipated cost will be prepared for you. This estimate is subject to change on the day of service depending on the complexity of your services. Should you have financial questions, please contact our office prior to your appointment. We are unable to finance self-pay accounts beyond 30 days. Delinquent accounts of 90 days will be assessed a finance charge of 18% and turned over to collections.
Insurance plans vary considerably. Although we call and verify your insurance coverage prior to your appointment, it is no guarantee of insurance payment. Should you wish to predetermine the benefits, you will need to provide precise information to our office so that we may assist you in obtaining predetermination of the benefits to which you are entitled.
We are not responsible for pursuing your insurance company to provide you insurance benefits; however, we will file a claim on your behalf and assist you in obtaining the maximum benefits available. Insurance account balances must be paid within 60 days regardless of insurance status.
Many insurance companies have specific provisions for you to be aware of. There are deductibles and coinsurance amounts that are an out-of-pocket expense. Also, insurance companies have yearly maximums, lifetime maximums and other procedures that are considered non-covered. You will be responsible for any services in excess to these limits. To be certain of your plans benefits or exclusions, we encourage you to contact your insurance company by calling the toll-free number on the back of your insurance card to verify your benefits.
